SHARED QUALITY IMPROVEMENT ACTIVITIES:
The Horizons Project is dedicated to providing prevention services to at risk youth and direct care to adolescents and young adults living with HIV, 13-24. It is the only comprehensive HIV/AIDS program for youth in Michigan. In 2010 Horizons was working with 147 youth in their program, 25 who were newly diagnosed. The Horizons Project started its youth retention QI Project in 2009 and continued to monitor youth retention throughout 2010. In fact, retaining youth in care is an ongoing effort. This QI Project reflects the program’s staff dedication.
